Do you want a personal CESNET CA 4 certificate?
Is your organization in the eduID.cz federation?
Get your certificate on the https://registry.cesnet-ca.cz/fed/ portal.

If you want server certificate or you are not in the eduID.cz federation:

Caution: Put in only name and password provided by the registration authority.
Other combinations of name/password definitely will not work.
